To assure the health and safety of all students and staff, daily screening for COVID-19 symptoms and possible exposure before arriving at school is required.

Complete the Home Symptom Screening for each day that your student attends in-person instruction and also if you will be visiting a PVUSD school site. Choose from the two buttons below, selecting either Elementary Schools or Secondary Schools, which will take you to a list to choose your student's specific school site. You will be directed to a form where you will answer two simple health symptom screening questions and submit.

You will need your student's ID number to complete the screening.

PVUSD's Home Symptom Screening form can be accessed on a Chromebook, using a phone or any device with Internet access.

If your student is feeling sick, please keep them at home and contact your student's school.
